Walk into any high school reunion and you’ll see it immediately: two people who are exactly the same age can look like they belong to different generations. One looks barely changed since graduation, the other looks a decade older. This isn’t random. Researchers have spent years studying “perceived age,” how old someone looks to others, and have identified specific genetic and lifestyle factors that explain much of this gap. This article breaks down what the science actually shows.
Perceived age is a real, measurable thing
Perceived age isn’t just a subjective impression. It is studied by scientists as a genuine biomarker of health. A landmark 2009 study published in The BMJ followed 1,826 Danish twins aged 70 and older. Independent assessors, including nurses and members of the public, estimated each twin’s age from photographs, without knowing their actual age (Christensen et al., 2009).
The findings were striking. Perceived age was significantly associated with survival, even after accounting for actual age, sex, and upbringing. Within twin pairs, the twin who looked older was more likely to die first, and this held true even after adjusting for physical and cognitive health (Christensen et al., 2009). The researchers also found that perceived age correlated with telomere length, a molecular marker of cellular aging found in white blood cells; the twin with longer telomeres tended to look younger and live longer (Christensen et al., 2009). In short, how old you look is genuinely tied to how your body is aging on the inside, not just a matter of appearance.
The role of genetics
A specific gene tied to how old you look. In 2016, researchers led by Manfred Kayser at Erasmus MC University Medical Center in the Netherlands studied the genomes of 2,693 elderly Dutch people and compared them to how old each person was perceived to look in photographs. They identified a gene called MC1R, already well known for its role in red hair and pale skin, as significantly linked to perceived age (ScienceDaily, 2016).
People carrying certain MC1R variants looked, on average, up to two years older than their actual age, independent of their skin color, sun exposure, or gender (ScienceDaily, 2016; TraitGen, n.d.). This was notable because MC1R also plays a role in inflammation and DNA repair, suggesting the gene affects aging through biological repair processes, not simply pigmentation (Time, 2016). Kayser noted that this was the first gene found to help explain why some people look older or younger than others, even after controlling for lifestyle and environmental exposures like sun damage (Time, 2016).
Genetics sets a real baseline, but it isn’t the whole story. Earlier research had already established that a person’s perceived age is shaped by a combination of genetic and environmental factors in roughly equal measure (ScienceDaily, 2016). Some estimates suggest biological aging at the cellular level is about 40% determined by genetics and 60% by lifestyle and environmental factors (TraitGen, n.d.), underscoring that while genes matter, they don’t fully determine how someone will age.
The lifestyle and environmental factors that matter most
Sun exposure is one of the biggest accelerators of visible aging. Ultraviolet (UV) radiation breaks down collagen and elastin, the proteins responsible for skin firmness and elasticity, leading to wrinkles, sagging, and uneven pigmentation over time. This effect, often called photoaging, is considered one of the most significant environmental contributors to how old a person’s skin looks, separate from chronological age.
Smoking accelerates visible aging. Tobacco use is consistently linked to earlier wrinkling, dull skin tone, and reduced skin elasticity, largely because smoking reduces blood flow to the skin and damages collagen.
Collagen naturally declines with age, and lifestyle affects how fast. Collagen is the structural protein that keeps skin firm and resilient, and its production begins declining as early as a person’s twenties (The Perfect Secret, 2026). Since collagen loss underlies much of visible facial aging, habits that support collagen and skin structure, such as sun protection and avoiding smoking, have an outsized effect on perceived age over time.
Diet, hydration, sleep, and alcohol all play a supporting role. A diet rich in antioxidants, adequate hydration, regular physical activity, sufficient sleep, and minimal alcohol intake are all linked to better skin elasticity and a more youthful appearance (Longevity by Nature, 2024). None of these factors work in isolation. Rather, they appear to work cumulatively, meaning consistent small habits over years matter more than any single treatment or product.
Chronic stress has a documented physical toll. Ongoing psychological stress has been linked to accelerated cellular aging, including effects on telomere length, the same molecular marker tied to perceived age in the Danish twin study (Christensen et al., 2009).
Why it’s rarely just one factor
Genetics can provide a natural head start or a disadvantage, but as the MC1R research illustrates, even a genetic predisposition toward faster visible aging can be worsened or offset by lifestyle choices (TraitGen, n.d.). Conversely, someone with genetically favorable skin can still age faster than expected through heavy sun exposure, smoking, or poor sleep. Perceived age is best understood as the visible result of an ongoing interaction between inherited biology and years of accumulated lifestyle exposure, not a fixed trait set at birth.
What this means practically
Since perceived age reflects real biological processes, not just surface-level appearance, the most evidence-supported ways to support a youthful appearance overlap heavily with general health advice:
- Protect skin from UV exposure consistently, since sun damage is one of the most significant and preventable accelerators of visible aging.
- Avoid smoking, given its well-documented impact on skin blood flow and collagen breakdown.
- Support collagen and skin health through a nutrient-rich diet, adequate hydration, and consistent skincare rather than sporadic intervention.
- Prioritize sleep and stress management, since both are linked to cellular aging markers like telomere length.
- Recognize that genetics set a baseline, not a destiny. Even people with a genetic predisposition toward looking older can meaningfully influence their visible aging through sustained lifestyle habits.
Bottom line
Why some people look younger or older than their chronological age comes down to a real, researched interaction between genetics and lifestyle. Specific genes, such as MC1R, have been shown to independently influence perceived age by up to two years, and perceived age itself correlates with molecular aging markers like telomere length, and even with survival. At the same time, environmental exposures such as sun damage, smoking, sleep, stress, and diet play a substantial, often larger, role over a lifetime. Looking younger isn’t purely genetic luck, and looking older isn’t purely a matter of neglect. It’s the accumulated result of both.
